Review of the acknowledgement transcript confirms that staff can receive accurate instructions for spaces that do not hold still long enough to be mapped. The error therefore should not be assigned to shelving, transit, or night staff alone.
Pending broader guidance, staff are advised to minimise descriptive language, maintain route discipline, and avoid assuming that closure of the file will also close the condition. This assumption has not tested well.
