A routine mismatch was identified between the declared location of the subject and the location from which its accompanying paperwork was recovered. Initial review suggested ordinary misfiling. Subsequent review did not support that conclusion.

At 21:10 the subject was signed into Lower Annex shelving. At 21:16 the same subject was confirmed as pending transfer from Corridor Stores. At 21:19 the transfer note was amended to indicate that no movement had yet taken place. The signatures are in order and no page appears to have been altered after submission.

Containment has not improved outcomes. When the file is moved, the route record remains inconsistent. When the route record is corrected, the associated shelf reference declines to remain singular.

For the moment, staff are asked to use the existing identifiers exactly as written and to avoid informal naming conventions, including descriptive substitutions. This has previously widened the review set.
